PEMPHIGUS VULGARIS / BULLOUS PEMPHIGOID
Tanım: Pemfigus vulgaris ciltte bir çok gevşek kabarcık veya görülebilir mukoz membran oluşumudur. IgG sınıfından anti-epidermik hücre membran antikorları (pemphigus antikorları) ciltte ve serumda mevcuttur. Büllöz pemfigoid ise ekstremitelerde, gövde ve vücudun diğer bölümlerinde göreceli büyük ve gergin kabarcıkların oluştuğu klinik tablodur. Serumda anti-bazal membran antikorları (BP antikorları) bulunur.
Description: Pemphigus vulgaris is the formation of visible mucous membrane or lots of loose blisters on skin. Anti-epidermal cell membrane antibodies from IgG class (pemphigus antibodies) are present in the skin and serum. As for Bullous pemphigoid, it is the clinical entity in which relatively large and tense blisters are formed in the limbs, trunk and other parts of the body. Serum contains anti-basement membrane antibodies (BP antibodies).
Gerekçe: Sebepsel maddelerin uzaklaştırılmasıyla (PV antikorları, BP antikorları) klinik semptomlarda iyiye gitme, steroidlerin azaltılması ve uzun dönemli remisyon hedeflenir.
Rationale: It is targeted imrovements in clinical symptoms, reduction of steroids and long-term remission by removal of the causative agents (PV antibodies, BP antibodies)

İşlenen Volüm: ECP: Mononükleer hücrelerden oluşan yaklaşık 200-270 ml mononükleer hücre ürünü, iki-basamaklı işleme metodunda, çift TBV işlemi ile elde edilen mononükleer hücreler toplanır ve işlenir.
TPD, DFPP, CF, IA: 1-2 TPH
Processed volume: ECP: Mononuclear cell product consisting of about 200 to 270 ml mononuclear cells in the two-step processing method, mononuclear cells obtained by double TBV process is collected and processed.
TPE, DFPP, CF, IA: 1-2 TPE

Uygulama sayı ve/veya süresi: ECP uygulanan hastalarda klinik cevap 2 ile 7 siklusdan sonra gözlenmiştir. Uygulanan toplam siklus sayısı 2-48 arasında değişmektedir. Bir sunumda; düşmüş otoantikor titresiyle birlikte %100 klinik yanıt sağlandığı bildirilmiştir. Takip süresi 4 ila 48 ay arasındadır. Çoğu hastada hastalık kontrol altına alınmıştır fakat sadece iki hasta oral sistemik ajanları sonlandırabilmiştir. ECP ile tedaviye klinik yanıt alınana kadar devam edildiği belirtilmiştir. TPD / DFPP / CF / IA işlemlerinde hasta antikor titresi ve klinik semptomlar takip edilmelidir. Yeterli ek immünsupresif tedavi ile deneme süresinin başarısız olması akabinde tedavi kesilmelidir.
Application number and/or duration : Clinical response has been observed after 2 to 7 cycles in patients treated with ECP. The total number of practised cycles ranges from 2 to 48. In a presentation; it is reported that 100% clinical response is achieved with decreased autoantibody titers. Follow-up duration ranges from 4 to 48 months. In most patients, the disease has been brought under control but only two patients have ended oral systemic agents. İt is reported that treatment with ECP had continued until the clinical response was received. Patients’s antibody titer and clinical symptoms should be followed in TPE / DFPP / CF / II processes. Treatment should be discontinued in case of failure of the trial period with sufficient additional immunosuppressive therapy.
