FOCAL SEGMENTAL GLOMERULOSCLEROSIS
Tanım: Fokal segmental glomeruloskleroz erişkin ve çocuklardaki böbrek yetmezlik nedenlerinin %15-20‘sini oluşturur. Karakteristik histolojik bulgusu normal bir glomerul komşuluğundaki bazı glomeruller de fokal skleroz alanlarıdır.
Description: Focal segmental glomerulosclerosis forms %15-20 of causes of renal failure in adults and children. Some glomerulis where they are characteristic histological symptom in a normal glomeruli neighbourhood are also the focal sclerosis areas.
Gerekçe: Renal fonksiyonda iyiye gitme, proteinüride iyiye gitme ve kolesterol düzeyinde azalma amacı ile yapılmaktadır. FSGS‘li hastaların serumu izole fare glomerülleriyle inkübe edildiklerinde derin albümin kaçağına sebep olan ve hastalık unsuru olarak nitelenen ve muhtemelen 30-50 kDa luk bir glikoprotein olan "geçirgenlik faktörü" nü içerdiği saptanmıştır.
Reason: It is performed with the purpose of decreasing at cholesterol level, getting beter at protein uria and at renal function. When FSGS patients’ serum were incubated with rat’s glomeruli, it was determined that it included ‘‘dissipation factor’’ causing albumin leakage and presumably a 30-50 kDa glycoprotein and qualified as disease factor .

Uygulama sayı ve/veya süresi: Toplamda, en az 9 işlem olacak şekilde 3 ardışık günlük işlemle başlanır. Ardından takip eden 2 haftalık sürede en az 6 işlem uygulanarak devam edilir. Tedavinin azaltılarak kesilmesine, vakanın durumuna ve proteinüri derecesine bakılarak karar verilmelidir. Klinik yanıtın zamanlaması oldukça değişkendir ve proteinürinin kontrol altına alınması haftalar-aylar sürebilir. Bazı hastalara idame tedavi olarak uzun dönem haftalık-aylık işlemler uygulanabilir. İşlemlerin başarısını önceden belirleyecek klinik ve laboratuvar parametre bulunmamaktadır.
Application number and/or duration: It is begun with 3 consecutive daily processes to be at least 9 processes. After, it is continued to perform at least 6 process during following 2 weeks., it should be decided to the termination of the treatment by reducing by looking at proteinuria level and situation of the case. Schedule of clinical response is so variable and it may take weeks-months to take proteinuria under control. As maintenance treatment, long-term weekly-monthly processes can be performed to some patients. There is not any clinical or labaratory parameter to predetermine the success of the processes.
