PHYTANIC ACID STORAGE (REFSUM) DISEASE
Tanım: Fitanik asit depo hastalığı otozomal resesif bir bozukluktur. Yetersiz alfa-oksidazdan ötürü hastalarda anlamlı fitanik asit metabolizma bozukluğu vardır. Fitanik asidin yıkılamaması neticesinde yağ hücresinden zengin dokularda ve plazmada birikir. Klinik neticeleri retinitis pigmentosa, periferik nöropati, cerebellar ataxia, sensorinöral sağırlık ve anosmi şeklinde nörolojiktir.
Description: Phytanic acid storage disease is an autosomal recessive disorder. The patients have the phytanic acid metabolism disorder because of the inadaquate alpha-oxidase. As a result of phytanic acid can not be catabolised, it accumulates in tissues rich in fat cells and plasma. Its clinical results are neurological as retinitis pigmentosa, peripheral neuropathy, cerebellar ataxia, sensorineural deafness and anosmia.
Gerekçe: Terapötik plazmaferez akut ataklar ve alevlenme dönemi ile birlikte idame tedavilerinde plazma fitanik asit düzeylerini hızlı şekilde düşürür. Bireylerde normal fitanik asit düzeyi < 33 µmol/L iken Refsum hastalığının semptomatik fitanik asit düzeyleri 700 ile 8000 µmol/L aralığındadır.
Reason: The therapeutic plasmapheresis reduces rapidly the plasma phytanic acid levels in maintenance treatments together with acute attacks and exacerbation period. While the normal phytanic acid level < 33 µmol/L at individuals, the symptomatic phytanic acid levels of Refsum disease are in the range 700 – 800.

Bazı vakalarda işlemlere idame olarak azalan sıklıkta haftalar/aylara kadar devam edilebilir. Tedavinin devam veya sonlandırılması kararı hastanın fitanik asit düzeyinin, klinik semptom/bulgular ile hastalığın alevlenmesi veya engellenmesi konusundaki takibi neticesinde verilir.
